I, the owner of the property to be treated for subterranean termites, approve the above treatment quote and herby acknowledge that the above mentioned Termite Control Sentricon Bait Station Agreement was explained to me by a representative of Anti-Pest Co. Inc. I also understand and agree that this property will continue to be monitored with the installed Sentricon bait stations, which will be invoiced monthly unless cancelled by calling the Anti-Pest office. Upon cancellation, bait stations will be removed from the property.*
