    There is no way to determine if our technician damaged the hot water heater located in the attic at the above listed property.

    For the sum of $100.00 (which is a refund of half the cost of the pest control service performed on 3/20/24), I, Taylor Andrews, being over 21 years of age, do hereby release, acquit and forever discharge Anti-Pest from any and all actions, causes of action and claims, demands, damages, loss of service, costs expenses and compensation on account of or in any way growing out of, any and all known and unknown personal injuries and property damage.

    Payment of this claim is in no way admission of liability on the part of Anti-Pest.

    This release contains the entire Agreement between the parties hereto, and the terms of this release are contractual and not a mere recital.

    I further state that I have carefully read the forgoing release and know the contents thereof, and I sign the same as my own free act.
