Pests Covered in This Agreement:

This service covers common household pests including German, American, Oriental, and Smokey Brown Roaches; Fire Ants (inside or within 50 feet of the home); Argentine, Carpenter, Pharaoh, and Rover Ants; Scorpions; Non-Poisonous Spiders; Ground Beetles; Centipedes; Millipedes; Earwigs; Sow Bugs; Pill Bugs; Crickets; Norway Rats; Roof Rats; Mice; and Ground-Level Stinging Insects on the structure (up to 10 feet high).

Pests not listed above may be treated for an additional charge. This service does not include termites or other wood-destroying insects, fleas, ticks, bed bugs, pantry pests, fabric pests, wildlife, birds, lawn or landscape pests, mosquitoes, or stinging insects above 10 feet, in the yard, or requiring ladder access.

Service Subscription Plan:

Includes 4 scheduled services per year, performed approximately every 90–120 days to provide consistent, year-round protection.
Your initial service includes a thorough treatment of both the interior and exterior of the home.
Ongoing services focus on the exterior perimeter, creating a protective barrier designed to keep pests from getting inside.
Spider webs will be brushed from the home at every regularly scheduled service.
If covered pests are noticed between regular services, we provide complimentary touch-up treatments in the affected area at no additional charge.
Fire ant mounds within 50 feet of the home are treated at no additional charge.
Pest + Termite Monitoring Package (Optional): When selected for an additional $10 per month, four termite monitoring stations will be installed around the home and checked for termite activity at each regularly scheduled service. If termite activity is detected, the customer may upgrade to the Sentricon® Termite Baiting System and receive a $250 discount on the installation.
Billing: This subscription is billed monthly to a credit card or ACH bank draft on file.

I, the customer, certify I have read and understand the terms of this Agreement. I understand that unpaid balances over 90 days may be sent to a collection agency, incurring fees up to 25% of the balance owed. If I cancel this subscription within one year of the start date, I will be charged a $100 cancellation fee or the remaining balance, whichever is less. I also understand that service pricing may be adjusted annually on the anniversary of this Agreement.*
